The Palette Fund’s commitment to create accessible Patient Navigation is expressed through our investment in an organization’s ability to provide Patient Navigation services and training.
Our Current Programs Include
Pancreatic Cancer Action Network
Pancreatic Cancer Action Network is a nationwide network of people dedicated to working together to advance research, support patients and create hope for those affected by pancreatic cancer and we are happy to announce that we have joined forces with the Pancreatic Cancer Action Network to augment their successful PALS (Patient and Liaison Services) program and launch The Patient Navigation Outreach Program. The Program aims to ensure that more patients and their caregivers have access to up-to-date information about the disease, are aware of and are connected to needed resources and services, and are equipped and empowered to make informed decisions about their care and treatment.
Harold P. Freeman Patient Navigation Institute
The Harold P. Freeman Patient Navigation Institute is the country’s first accredited Patient Navigation Institute, originally affiliated with the Ralph Lauren Cancer Center in Harlem. They offer three kinds of programs; online training; a three-day training course on-site (open to anyone); and customized training. The Palette Fund’s grant will help launch the first-ever Patient Navigation Institute for people living with HIV/AIDS in conjunction with One Living.
New York Zen Center for Contemplative Care
The Palette Fund will provide a grant to the New York Zen Center for Contemplative Care, which provides compassionate care to the sick and terminally ill and creates a supportive and nurturing environment for people to consciously face their illness and/or end of life journey.
One Living
One Living is the nation’s first online community for those living with HIV/AIDS that integrates social networking, empowering content, and useful information. The site seeks to create a supportive community and increase the accessibility of the best treatment and wellness information for people living with HIV/AIDS. Palette’s grant supports the launch of this powerful website as well as the integration of the Patient Navigation partnership with the Harold P. Freeman Institute.
Bailey House
For the past 26 years, Bailey House has been a leading AIDS organization in New York City. They were the first in the nation to provide housing to homeless men, women and children living with HIV/AIDS. Palette’s grant will expand the organization’s patient navigation services.
Smith Farm Center for Healing and the Arts
Smith Farm Center has expanded the concept of patient navigation by developing an integrative navigation model that advocates the use of both medical intervention and complementary supportive modalities in addressing cancer to facilitate healing on all levels: physical, mental, emotional and spiritual. The Center offers a unique, innovative patient navigation training program that equips navigators with the knowledge, skills and resources to successfully establish an integrative cancer navigation practice.
This intensive five-day program, taught by experts in such specialties as oncology care, the healing arts, cancer survivorship, palliative care, nutrition, and stress reduction, offers information and tools that navigators can use to provide integrative navigation to patients throughout the entire treatment continuum.
The training prepares navigators to offer individuals facing cancer education, advocacy, support, guidance and instruction and companionship that improves quality of life and contributes to more patients successfully completing treatment. Following the training, participants are able to:
- Offer psychosocial support to individuals and their caregivers
- Facilitate access to health care and integrative therapies
- Reduce the stress of living with cancer
- Engage patients in taking ownership of their health and wellbeing
- Empower patients with knowledge and skills to enhance their treatment experience and improve their quality of life
- Facilitate the physical, emotional and spiritual healing of each individual
- Learn and use basic, proven integrative mind-body therapies with clients
The Palette Fund partners with Smith Farm Center to provide scholarship assistance to individuals to attend their Navigation Training and is currently working on a grant that would enable the Center’s Navigation Training to expand outside of the DC area.
